Consent to Discuss at WMS

At West Meon Surgery we believe in people having the right to privacy when it comes to their healthcare.

As such, we will not discuss a person’s healthcare with anybody without permission. This includes married couples or parents with children who are old enough to decide if they want to have an advocate speak on their behalf.

This outlook also applies to the ordering of medication through our dispensary.

As part of this ongoing approach, we have introduced a form which allows you to quickly and concisely let us know if you would like somebody to discuss things on your behalf or order medication.

You can download it below or collect it from us in the surgery. Once we have the form returned to us, we will upload it to your notes.

If you have multiple carers who help you day to day, you will need to use a different form found below. We would like to know the names of each of your carers along with whom they are employed by, allowing us to make sure that we’re only speaking to people who are protecting your best interests.
